In the first appointment there are some forms that are a necessary part of getting started. You will normally receive an email from me shortly after our Intake call and before our first appointment with an Important Information for New Clients Form. It will contain links to the below linked forms.
Prior to the session, please review the General Information Form and the Informed Consent Form (some clients even print the Consent form and bring it with them to the first session). When we meet I will need to go over the Consent Form with you and both of us will sign it. Please note: If you are attending as a couple, each person attending will have to individually review and sign a separate Informed Consent Form.
The more familiar you are with both the above forms the more quickly we can move through their content and get to talking about the things that matter to you.
You will also need to print off and fill out the Client Information Sheet and bring it completed to the first appointment. Please note: If you are coming as a couple, each person attending will have to bring their own completed copy of this form. After the Consent Form has been completed I will ask you to give me your completed Client Information Sheet. This handout is 4 pages and involves information that is needed for your file as well as helping you mentally prepare for the session. This form takes about 15 minutes to complete. I'd much rather spend the time we have talking to you than filling a form so please come with it completed.

If you need me to release information about you to someone else you'll need to print off, fill out and fax or bring in the Consent to Release Confidential Information Form.
If you need me to obtain confidential information about you from someone else you'll need to print off, fill out and fax or bring in the Consent to Obtain Confidential Information Form. If you need me to obtain confidential information about you from someone you will likely also need to complete the above Consent to Release Confidential Information Form in order for me to disclose that you are my client in the first place.
Online Sessions by Video Conference
If you have already seen me at least once for a face to face session you may have been offered the option of scheduling an online video conference appointment with me. This isn't a fit for everybody or for every issue but for some clients it's a convenient way to have sessions and access support.
If you've been offered this option then you will need to make sure you have signed the Consent Form for Online Sessions and sent it to me before that first video conference sessions starts. All the information about the secure software I use is linked in this form so please take the time to review it and ask questions.
If you and I have decided to book a session by video conference then you can take care of payment by online Interac eTransfer (preferrable) or by completing and submitting prior to session this Credit Card Authorization Form.
